Appointments

Initial diagnostic assessments are 60 minutes. 

The length of follow-up visits will be determined by the treatment plan.

Missed Appointments & Cancellations

If you need to cancel or reschedule, please notify our office at least 24 hours in advance. Missed appointments or late cancellations WILL incur the full appointment fee. Three or more repeated missed appointments may affect your ability to schedule future visits.

Health Insurance

I current accept Blue Cross Blue Sheild PPO. I am in the process of credentialing with Aetna and Optum/UHC Insurance providers in order to provide greater access to care.

At this time, I am not enrolled with Medicare or Medicaid.

If I don't take your insurance, you may be eligible for out-of-network reimbursement from your insurance provider. I am happy to provide a superbill for this.

Payment of Fees

Payment is due at the time of service. We accept major credit cards and debit cards.
